Player Registration Information

Competitive Player Registration Deadline is August 24th 2007

Forms to be Returned (Current Players) for the '07 Season:

1.    Individual Member Registration:  Please fill in as complete as possible.

2.    Consent to Treat:  Please fill out completely including insurance company and policy number and sign.

3.    Liability Release:  Please sign and date.

4.    Government Issued Copy of Birth Certificate.  We cannot accept hospital records.
